For your convenience, we have included important patient forms on our site. Save time during your appointment by printing off the forms and filling them out beforehand. We work with all insurance providers, but are in network with Metlife, Cigna,Dental Health Alliance/Assurant, Humana PPO, Ameritas/Principal, City of Amarillo, Aetna, Delta Dental, Humana, GEHA/Drs. Alliance, Dentegra, and Connection Dental. For patients who are 21 years of age and younger, we accept Medicaid and Oklahoma SoonerCare. If we are not covered by your insurance provider, we can still bill them. Our office offers flexible financing options through CareCredit and Amarillo National Bank.

Bryan F. Bailey, DDS, an oral and maxillofacial surgeon in Amarillo, Texas, specializes in a variety of oral procedures, including wisdom teeth removal, tooth/teeth extractions, and various dental implant procedures. Dr. Bailey’s approach to facial trauma, reconstructive surgery, bone grafting and implants is truly unique and is focused on fully restoring oral function. He holds a license for IV and general anesthesia and is a member of the Texas Dental Association, Panhandle District Dental Society, American Association of Oral and Maxillofacial Surgeons, and the American Dental Association and the American Board of Oral & Maxillofacial Surgery.
